COMMUNITY HEALTH NURSE II (#24350, 24351)
Good Samaritan Hospital in Cincinnati, OH
FT & PT, days & eves

This position assists in the execution of community health fairs looking at results obtained from on site screenings such as glucose, lipids, heel dexa scans, BMI, and blood pressures. Nurse:

*discusses the results of the screenings with the participant in relation to her health status and provides education to help improve the health of the individual
*follows up to ensure that patients with abnormal screenings are seen by a health care provider and communicates as needed with the health provider
*will assist the Community Health Educator as needed with calls, data collection, and other specified duties
*communicates with and assists the Program Coordinator as needed
